Major reality TV shakeup -- Carole Radziwill is heading back to "The Real Housewives of New York City" ... but pump the brakes, she’s not reclaiming her apple full-time.

TMZ has confirmed Carole -- who held court from S5 through 10 -- is returning as a "friend" for Season 16, with cameras firing up in NYC this week.

During her OG run, Carole was no wallflower ... locking horns with Bethenny Frankel, Ramona Singer, Sonja Morgan and Luann de Lesseps in some of the franchise’s messiest moments.

TMZ Sports has obtained the dispatch audio from Dominiq Ponder's fatal crash ... with responders detailing the scene following the tragic accident that took the Colorado quarterback's life.

The audio, from Broadcastify, is from early Sunday morning ... when the 23-year-old's 2023 Tesla was found near Baseline Road in Boulder County.

The dispatchers are heard relaying information ... with responders initially believing there was a fire on the side of the road from a blown-up transformer. It was later discovered that the flames were caused by a car crashing into the pole.

Forget hot takes -- "The Dan Patrick Show" had actual flames coming out of its studio while it was live on the air.

Legendary broadcaster Dan Patrick was mid-show on Monday morning -- casually taking a call from a listener named Adam -- when the fire alarms in the Man Cave suddenly started blaring.

Savannah Guthrie and family members were filmed outside their mother Nancy's Tucson, Arizona home this morning ... taking in a growing memorial for their mom one month into her search.

Watch the clip ... Savannah, her sister Annie, and brother-in-law Tommaso embraced as they laid flowers for Nancy. The memorial consisted of many bouquets and also signs with loving messages, along with what appears to be Nancy's "Missing" poster.
